REGISTRATION MENU OPTIONS

 

 

Authorization

 

This option has two purposes: to create a referral request to the insurer by a primary care physician (PCP) for approval for a patient to see a specialist, or to create an authorization request to the insurer to approve service for a member based on review of the member's plan benefits and/or review of the service for medical necessity.

 

This screen contains many Text Box fields. To exit Text Box fields either use the [Control] [Tab] keyboard combination two times, OR click on another field.

 

1. To add either a new referral request or new authorization request in Intellect, go to Registration --►Authorization --►Add

 

 

The Add AUTHORIZATION screen opens:

 

 

2. Account                                     

 

2.1 Type the Patient Account number for which the referral or authorization is to be requested.

 

2.2 This field is mandatory and must be completed. If left blank, an error message displays:

 

 

Press the [Ok] button to return to the <Account> field.

 

2.3 Enter the Account Number, OR type up to 6 characters of the last name and press the [F2] key to display the Searching screen. To select the number from the list, either highlight the record and press the [Enter] key, OR double-click on the record. 2.4 Intellect completes the following area of the Referral Authorization Request form from the patient registration screen. Information must be completed during Patient Registration.

 

 

 

3. Name                                         

 

3.1 The patient name displays Last, First. This is a read-only field and may not be modified.

 

4. Referred To                               

 

4.1 If this is a request for the patient to see an outside specialist, type a Utility --►Referring <Referring Code>.

 

4.2 Enter the Referring Code, OR type up to 6 characters of the description and press the [F2] key to display the Searching screen. To select the code from the list, either highlight the record and press the [Enter] key, OR double-click on the record. Alternately, after clearing the field, press the [F2] key twice to view the entire list of codes. See Introduction to Intellect --►Intellect Characteristics and Features -- Searching for more information on the Searching screen.

 

4.3 Intellect completes the 'Referred To' and 'Telephone Number' information on the Referral Authorization Request form based on the completed information on the Utility --►Referring screen.

 

 

5. Referred Name                          

 

5.1 The referred name displays Last, First as entered on the Utility --►Referring screen. This is a read-only field and may not be modified.

 

6. Phone                                        

 

6.1 The referred phone number displays as entered on the Utility --►Referring screen. This is a read-only field and may not be modified.  

 

7. Type of Referral                        

 

7.1 Enter the appropriate Referral type. For example: Routine, Urgent, Emergency, Retro, or Express.

 

7.2 Intellect completes this area on the Referral Authorization Request form:

 

 

8. Requested By                             

 

8.1 Type the Utility --►Provider <Provider Code> for the provider requesting the referral or authorization.

 

9. Requested Name                       

 

9.1 The Provider name displays. This is a read-only field and may not be modified.

 

9.2 Intellect completes the Referral Authorization Request form along with the provider's phone number, as entered on the Utility --►Provider screen.

 

 

10. Clinical Symptoms                  

 

10.1 Enter the observed clinical symptoms.

 

10.2 Intellect completes the Clinical Justification section of the Referral Authorization Request form.

 

 

11. Physical Findings                    

 

11 1 Enter the actual physical results of exams, test, x-rays, labs etc.

 

11.2 Intellect completes the Physical Findings section of the Referral Authorization Request form.

 

 

12. Medication(s) Tried                 

 

12.1 Enter the history of any medication used to treat this condition.

 

12.2 Intellect completes the Medication Tried section of the Referral Authorization Request form.

 

 

13. Date Of Service                       

 

13.1 If requesting a specific date of service, type the date. 

 

13.2 This field may be completed after the request is approved and a date has been scheduled.

 

14. Diagnosis                                

 

14.1 Enter the Utility --►Diagnosis --►Diagnosis <Diagnosis Code>.

 

14.2 If entering an invalid code, a prompt displays:

 

 

Press the [OK] button to return to the <Diagnosis> field.

 

14.3 Enter the Utility --►Diagnosis --►Diagnosis <Diagnosis Code>, OR type up to 6 characters of the description and press the [F2] key to display the Searching screen. To select the code from the list, either highlight the record and press the [Enter] key, OR double-click on the record. See Introduction to Intellect --►Intellect Characteristics and Features -- Searching for more information on the Searching screen.

 

15. ICD10                                       

 

15.1 Intellect displays both the Utility --►Diagnosis --►Diagnosis <ICD10 Code> and <Description> from the <Diagnosis> code entered above. This is a read-only field and may not be modified.

 

15.2 Intellect completes the Diagnosis section of the Referral Authorization Request form.

 

 

16. Procedure/Service Requested  

 

16.1 Enter the Utility --►Procedure --►Procedure <Panel Code> for the procedure requested.

 

16.2 If entering an invalid code, a prompt displays:

 

 

Press the [OK] button to return to the <Procedure/Service Requested> field.

 

16.3 Intellect completes the PROCEDURE/SERVICE REQUESTED area of the Referral Authorization Request form.

 

 

17. CptCode                                  

 

17.1 This field displays the Utility --►Procedure --►Procedure <Code C> for the procedure/service requested. This is a read-only field and may not be accessed for modification of data.

 

17.2 Intellect completes the CPT CODE area of the Referral Authorization Request form.

 


NOTE: The Procedure/Service Requested and CptCode fields are repeated 4 times on the screen.  The instructions for completing these fields are the same as for fields 16 and 17 described above.

 

18. Facility                                    

 

18.1 Enter the Utility --►Facility <Facility Code> for this requested procedure.

 

18.2 If entering an invalid code, a prompt displays:

 

 

Press the [OK] button to returns to the <Facility> field.

 

18.3 Enter the Utility --►Facility <Facility Code>, OR type up to 6 characters of the description and press the [F2] key to display the Searching screen. To select the code from the list, either highlight the record and press the [Enter] key, OR double-click on the record. See Introduction to Intellect --►Intellect Characteristics and Features -- Searching for more information on the Searching screen.

 

19. Facility Name                         

 

19.1 Intellect displays the Utility --►Facility <Name> from the <Facility Code> entered. This is a read-only field and may not be modified.

 

19.2 Intellect completes the SERVICE TO BE PROVIDED AT section of the Referral Authorization Request form.

 

 

20. Approved                                

 

20.1 Type the approval code or name, if available.

 

21. Approved Date                        

 

21.1 Type the approved date, if available.

 

22. Comment                                

 

22.1 Type any additional comments.

 

22.2 Intellect completes the General Comments section of the Referral Authorization Request form.

 

 

23. Length of Stay                         

 

23.1 If in-patient treatment is necessary, type the amount of authorized/requested time.

24. Form Code                              

 

24.1 Not currently available.

 

25. When the information is completely entered, select one of these options:

 

 

25.1 Click [Add] to save the record, clear the form, and return the focus to the <Account> field.

 

25.2 Click [Clear] to clear the form without saving and return the focus to the <Account> field with the current account number active.

 

25.3 Click [Exit] to close the form and return to the main Intellect screen without saving.

 

26. To print, go to Registration --► Authorization --► Search
