ONLY the HHA or Hospice’s Medicare Facility is entered, thus use <Remark> to reflect which HHA or Hospice this Facility Code is being used for.

Add an encounter for the patient. Enter the applicable qualifier in the <Place of Service> field. To date, applicable qualifiers are: ST (speech therapy), PT (physical therapy), AI (home health aid), SN (skilled nursing), OT (occupational therapy), MS (medical social worker).
Enter the projected number of visits on the <Authorized Visits> field.

Post charges as usual, attaching the related encounter to that charge session via the <EN#:> field.
